Our family are in the process of relocating my brother to a new care home. He will be moving away from home that has not been providing sufficient care, but also moving closer to family.
We have had a Best Interests meeting with social services and the care home, and the move has been agreed by social services, in principal.
One of the requirements for sign off is for us to prepare a Transition plan which incorporates a back up plan if my brother doesn’t settle into his new home.
Have any of you been through this process, or have experience of what to include in a Transition plan?
My brother is 54 years old, lacks mental capacity, has severe learning disabilities and autism. He functions around the level of a toddler.
Any help is gratefully appreciated.
